Immunization Requirements
Incoming Students
All incoming students must have documentation of the following immunizations:
- Tetanus/Diphtheria (Td) booster within the last 10 years
- Two Measles, Mumps, Rubella (MMR) in a lifetime
- PPD/TB skin test with documented results within the past 12 months
International Students
All international students must have documentation of the following immunizations:
- Primary series of Tetanus, Diphtheria, and Pertussis (Tdap)
- Two Measles, Mumps, Rubella (MMR) in a lifetime
- PPD/TB skin test with documented results within the past 12 months
School of Nursing
All School of Nursing students must have documentation of the following immunizations*:
- Tetanus, Diphtheria, and Pertussis (Tdap) booster if Td is more than two years old
- Two Measles, Mumps, Rubella (MMR), in a lifetime
- PPD/TB skin test with documented results within the past 12 months
- Hepatitis B series
- Optional: Hepatitis A and Meningitis
- Titers for Measles, Mumps, Rubella, Hepatitis B, and Varicella may be required depending on clinical site requirements.

Department of Physical Therapy
All Department of Physical Therapy students must have documentation of the following immunizations*:
- Tetanus, Diphtheria, and Pertussis (Tdap) booster if Td is more than two years old
- Two Measles, Mumps, Rubella (MMR), in a lifetime
- PPD/TB skin test with documented results within the past 12 months
- Hepatitis B series
- Titers for Hepatitis B, Varicella, and MMR

Walkabout
All individuals participating in the Walkabout Program must have documentation of the following immunizations:
- Tetanus/Diphtheria (Td) booster within the last five years
- PPD/TB skin test with documented results within the last 12 months
